Young Persons Social Prescribing
“Social prescribing” for young people is aimed at young people aged 11-18 years; and up to 25 years if the young person has additional needs. This involves the steps on the right.
Hartlepool’s young people’s social prescribing project is led by Hartlepool Sport CIC and the West View Project and delivered by a social prescribing link worker Sophie Bainbridge and overseen by a steering group led by Hartlepool Sport CIC’s Calvin George and supported by Hartlepool Community Trust’s Juli Simons. Membership of the steering group is open and this post funded by the Hartlepool Health Primary Care Network.

Step 1.
Identifying young people whose lives are affected in various ways, such as experiencing anxiety, social isolation, excessive worrying, low self- confidence or some combination of these. Typically, referrals come from schools, parents, social workers, early help and young people themselves.However, there have been many other routes into the project such as group work and wellness programs.

Step 2.
Building rapport with the young person to establish trust, to understand their interests and explore the current perceived barriers to living a more fulfilled activity filled life. This is done through 1:1 conversations with the referrer, education and their family/carers if suitable.

Step 3.
Navigating the young person to activities and experiences that are nurturing and fulfilling. This is tailored to the individual needs of the young person and serves as an "enabler" to future integration with a wider support circle post completion of these interventions . The YP Social Prescriber leans on resources and relationships with local clubs and community organisations to provide a myriad of options while carefully locating gaps in provision and feeding back to the capacity building team on the steering group.

Step 4.
Financial support for these experiences is available initially to remove any barriers. This also may include help with transportation, clothing , footwear and assistance if deemed appropriate.
